How does a patient and/or patient-authorized representative view, download and transmit patient health data from Patient Fusion?

Patient Fusion, Practice Fusion’s patient portal, allows patients and/or patient-authorized representatives to view, download and transmit patient health record data directly from the patient portal.

Practice Fusion provides the ability for your doctor to give you and/or your patient-authorized representative access to your medical records via Patient Fusion, our patient portal. As a patient and/or patient-authorized representative, you can view, download and transmit data that has been shared by the Practice Fusion EHR user. Most health information is shared as it is recorded in the EHR, but some information (such as laboratory results) requires that your healthcare provider explicitly indicate in the EHR that it is ready to be shared with you and/or your patient-authorized representative in the PHR.

From the portal, you and/or your patient-authorized representative can download the patient medical record locally to your computer or to an external storage device such as a USB drive in a human-readable CCD file. Alternatively, you and/or your patient-authorized representative can email your records to another recipient using any email address with the option to encrypt and password protect the attachment.

Most of the patient’s health record information is viewable in the Overview section of the patient portal. More detailed information can be seen by selecting the data category in the left menu of the portal (Graphic 1).

How do I view my records? 

  1. Log into your Patient Fusion account using the email address provided to the Practice Fusion EHR users office at the time of portal invite and password you created at time fo set-up.
  2. Select Health Records on the left-hand navigation bar as displayed in Graphic 1, above. This will take you to an overview of your health records.
  3. From the Overview page, you can select a date range of the records you wish to see an overview of using the Select a Date Range calendar at the top of the page (see Graphic 2 below).
  1. You can choose to view records for the last 7 days, 30 days, 60 days, 90 days, 1 year, or can select a custom date range. If no date range is selected, all available data will be displayed.
  2. Not all sections can be filtered by date. Allergies, Care Team, Health Concerns, and Social History will always be downloaded, regardless of date filter.

How do I download my records? 

  1. Log into your Patient Fusion account using the email address provided to the Practice Fusion EHR users office at the time of portal invite and password you created at time fo set-up.
  2. Select Health Records on the left-hand navigation bar as displayed in Graphic 1, above. This will take you to an overview of your health records.
  3. From the Overview page, you can select a date range of the records you wish to see an overview of using the Select a Date Range calendar at the top of the page (see Graphic 2 above)
  4. Click the blue Export button in the top right corner as displayed in Graphic 3 below.

5. Using the pop-up box to export your health record, shown in Graphic 4, choose to download the medical record locally to your computer or to an external storage device such as a USB drive in a human-readable CCD file. Click Continue.

6. Check the sections you want to include in the downloaded Health Record document. You can choose to include: Allergies, Assessments & Plans, Diagnoses, Encounters, Goals, Health Concerns, Immunizations, Lab Results, Medical Equipment, Medications, Procedures, Referrals, Social History, and Vital Signs.

7. After you make your selections regarding included data, choose to download your data file as an XML file (which is machine-readable) or an or HTML file (which is human-readable). (See Graphic 5, below.)

a. After selecting which type of file you would like to download, the file will immediately download to the computer you made the request from.

How do I transmit my records? 

  1. Log into your Patient Fusion account using the email address provided to the Practice Fusion EHR users office at the time of portal invite and password you created at time fo set-up.
  2. Select Health Records on the left-hand navigation bar as displayed in Graphic 1, above. This will take you to an overview of your health records.
  3. From the Overview page, you can select a date range of the records you wish to see an overview of using the Select a Date Range calendar at the top of the page (see Graphic 2 above)
  4. Click the blue Export button in the top right corner as displayed in Graphic 3 above.
  5. Using the pop-up box to export your health record, shown in Graphic 6, choose to transmit via email, then click Continue.

6. On the next page enter the recipient's email address in the first box, and verify the email address in the second box to Transmit via email.  

7. Select the sections you want to include in the Transmitted Health Record (see Graphic 7 below). You can choose to include: Allergies, Assessments & Plans, Diagnoses, Encounters, Goals, Health Concerns, Immunizations, Lab Results, Medical Equipment, Medications, Procedures, Referrals, Social History, and Vital Signs.

8. Check the box to Encrypt and password protect if you wish to send an encrypted copy. When sending your health record via encrypted email attachment, you must share the password with the recipient in order for them to access the file upon receipt.

 9.Create and confirm your password that has at least 8 characters.  (See Graphic 7 below.)

10. After creating and confirming the password, click Transmit health record summary as displayed in Graphic 8.

How will the recipient view my record?

  1. The recipient of the health record summary will receive an email from noreply@patientfusion.com. The subject of the email will be “Clinical Information from [Patient Name]”.
  2. An example of the email that is sent when you do not encrypt the medical record attachment is shown in Graphic 9 below.

4.  After downloading a password protected document, the recipient will be prompted to enter a password (see Graphic 11 below). You will need to provide them with the password you protected the file with.

5.  Both options will prompt the recipient to download a ZIP file containing the clinical record in both HTML and XML formats.
