How do I create a unique email address for logging into Patient Fusion?

Patients and/or patient-authorized representatives will need a unique email address for logging into the Patient Fusion account on record to view, download and transmit, medical data contained in the health record.

The email address provided to your doctors office at time of patient portal invite will be the email address you use to log into Patient Fusion. Patient Fusion will use this email address to send communications including password reset emails and important updates about the patient portal. If you already have a unique email address, you don’t need to create a new one.

If you wish to create a new unique email address, you can use web-based email services to create a new free email address. Below are links for creating email accounts with some popular web email services.

Frequently Asked Questions

What is a unique email address?

A unique email address is an email that only you can access (i.e. is not shared with other people). Patient Fusion will use this email address to send you password reset emails and important updates about the patient portal.

Can I change the email address I use to log in?

Yes. Navigate to Account Settings from the left hand navigation menu. Click the small blue Edit button next to the Account email section to make changes. Use an email address you check regularly and can access anywhere.

What will happen to my username once I switch to email login?

Your username will no longer be used to log in or perform any actions in your Patient Fusion account. A unique email address will be the only means to identify you and/or your patient-authorized representative.

I have multiple usernames for a single email address. Can I keep my accounts separate, or will I need to consolidate?

Usernames will no longer be accepted as a method to access your Patient Fusion account. If you wish to keep multiple Patient Fusion accounts separate, you will need a separate unique email address for each individual account or you can merge your accounts so all records are associated and viewable with one account and one login email address.
