How can I reset my account?

To have your account reset, you'll need to contact your doctor and request this. Once your doctor has reset your account, you'll receive a new registration email, titled "Your Health Records", with a new registration link set up your account. You'll also need to request the new PIN from your doctor to verify your identity when signing up. You can then complete the registration process again.

Please note that you will need to choose a username other than your previous username (sometimes this is your email, so you'll need to come up with a new username).
