What is available in my Patient Fusion account?

The Patient Fusion patient portal is your source for up-to-date health records shared with you by your doctor, including:
  • Medications
  • Diagnoses
  • Allergies
  • Immunizations
  • Lab test and results
  • Procedures
  • Care plans
  • Appointments
Please note, all information contained within your Health Center account must be documented and intentionally shared by your doctor's office. Please contact your doctor directly if you're noticing a discrepancy in the information contained in your account. Additionally, only the information documented by the doctor who invited you to register your patient portal account will be available in your account (rather than all previous medical records).

The Health Records section is where you'll find health information that has been shared by your doctor.

The Appointment section will include upcoming appointments, past visits and canceled appointments. You can also book, or request to reschedule and cancel appointments within this section. For more information, please see the links below:
