The default username is the email address your doctor used to grant you access to your health records.
Requesting username retrieval
1. To request an automatic retrieval of your username, visit my.patientfusion.com and click "Forgot username."
2. You will then be prompted to enter your email address and date of birth.
3. If the information entered matches our records, an email containing your username will be sent to the email address your doctor's office has on file for you.
If the automatic retrieval process could not be completed, please contact your doctor’s office and ask to for your username. If you've successfully registered your account, they will have the username you chose on file. They will simply need to access this information in your chart. Please note, we cannot retrieve your username on your doctor’s behalf.
